Use the statewide Office of Open Records form. Give the requester’s name and contact address, identify the person and commitment, and ask for specific existing admission, discharge, status, destination, or booking-photo fields. The City ordinarily responds within five business days, either finally or with an interim response. A statutory reason may support an extension.
